The first step is to complete the required Self-Determination orientation through your Regional Center or the State Council on Developmental Disabilities (SCDD). This orientation is mandatory before we can begin working together.
To be fully prepared, it’s also helpful to review your current Regional Center services and gather key documents, such as your Individual Program Plan (IPP) and your most recent expenditure report. Having these ready will make your Discovery Meeting more productive and focused on your goals.
To prepare for your transition Individual Program Plan (IPP) meeting with your Regional Center, you’ll want to come organized and ready to clearly present your needs, goals, and budget planning. Here are the key items to have:
1. Completed SDP Orientation Certificate
You must complete the Self-Determination orientation through your Regional Center or the State Council on Developmental Disabilities. Bring proof of completion.
2. Your Current IPP
Have a copy of your most recent IPP so you can review existing services and identify what you want to keep, change, or remove.
3. Person-Centered Plan (PCP) (if completed)
This is highly recommended and often required. It outlines your goals, preferences, and vision for services under SDP.
4. Recent Expenditure Report / Budget Information
This helps determine your individual budget, which is typically based on your prior 12 months of services.
5. List of Desired Services & Supports
Write out exactly what you want in SDP, including:

✔ New services or supports you want to add
✔ Any changes in providers or service types
6. Provider Ideas or Options
If you already have providers in mind (including an Independent Facilitator), bring that information. You are allowed more flexibility in choosing providers under SDP.
